Terri Roberts Massage Therapy is committed to protecting and respecting your privacy. This policy contains important information about how I use and treat your personal data in line with compliance with the Data Protection act 2018.
This policy helps you understand what information I collect in relation to my services and how I collect, use, share, retain and safeguard your personal date as well as your rights on what I do with your data.
Information I collect-Personal Details.
You may give me information over the phone, via email, by SMS text or via my Facebook business page when you first contact me.
This includes collecting your name and contact number, email and brief details of why you are contacting me. This information gathered is needed to ensure I can contact you regarding your appointment and select the best service and length of appointment necessary.
At your appointment I collect and process the following data about you via a consultation form. Name, demographic information, contact numbers, email address, next of kin and contact number, health and medical background.
If you object to giving this information, I may not be able to safely provide you with my services. The initial assessment and treatment notes are added to your data which may include data about your medical background, height, weight, posture, injuries and any other findings during treatment.
Where do I keep your data.
All treatment forms are stored in my premises, in a locked filing cabinet.
No information regarding your treatment is currently entered online, should this change, this will take place on a password controlled laptop, only I would have access to these records.
How data is protected
I take appropriate steps to protect the confidentiality, integrity, availability and authenticity of your data.
Regular data audits will be undertaken and an annual review of this policy.
Record keeping
In line with my insurance providers requirements, the records are kept for at least 7 years following the last visit. In the case of treatments to minors the records are kept for 7 years after they reach the age of maturity (18 years). After this retention period the records are destroyed.
Sharing
I do not share any of your personal date with 3rd parties unless consent has been given to pass on details to an osteopath, chiropractor or GP in case of further referral. I would not do this without your permission.
Your rights
You have the right to:

· To receive an electronic copy of your personal data
If you request the deletion of your personal data, I may be required to retain some data for taxation, legal and account purposes but personal identifiable data will be deleted.
Complaints
If you are not happy with any aspect of my record keeping or confidentially of your data you have the right to complain.
